Initials of surveyor and name of surveyor.

Enquiry source

Enquiry source code and description of enquiry source.

Rejection Fields:

Date	Date quotation was rejected and day of week.

Rejecter

Initials of rejecter and name of rejecter.

Reason

Rejected reason code and description of rejected reason.

N/A	Not applicable.

Credit card

Refund credit card surcharge.

Debit card

Refund debit card surcharge.

Save	Close window and save any changes you have made.

Close window.

TIP

If the surcharge options (N/A, Credit card, Debit card) are disabled (see image above), this means no surcharge can be refunded.
